Acute Hospital at Home Services Under the §1135 Waiver

The Centers for Medicare & Medicaid Services (CMS) acute hospital at home waiver (waiver) is an initiative that allows eligible patients to receive acute hospital-level care in their own homes (referred to as “hospital at home” for the purposes of this article), with remote monitoring and daily visits by health care professionals. The initial aim of the program was to increase hospital capacity, reduce COVID-19 exposure, and improve patient outcomes. In short, the waiver waives §482.23(b) and (b)(1) of the Medicare Hospital Conditions of Participation (COPs) requiring nursing services to be provided on premises 24 hours a day, seven days a week and for the immediate availability of a registered nurse for care of any patient.
