Health care providers, social service providers, health policy experts, researchers, and patient advocates have known for decades that social determinants of health (SDOH) directly drive poor health outcomes and high-cost care. SDOH are the social and economic conditions in which people are born, live, and work. Health behaviors and SDOH account for 80% of health outcomes across populations.
